In this outpatient setting, you'll use the nursing process to assess, plan, implement, and evaluate care, while ensuring patients and their families are actively engaged in every step of their healthcare journey.You'll coordinate care across departments, advocate for patient rights, and maintain confidentiality with professionalism and compassion. Your role includes developing personalized care plans, identifying and documenting educational needs, and supporting seamless communication through tools like SBAR during patient handoffs. You'll participate in physician rounding, contribute to interdisciplinary collaboration, and stay current with certifications, policies, and evidence-based practices.In addition to clinical responsibilities, you'll support continuous improvement efforts, mentor support staff within their scope of practice, and promote a culture of learning and excellence.
